The strange icon is an item obtained from the Barrows chest during the miniquest His Faithful Servants. It can only be obtained from the chest after defeating all six Barrows brothers, and is given to the Strange Old Man to complete the miniquest.
Looting the icon while not having enough inventory space will cause the icon to be destroyed, and you essentially have to restart the quest in order to obtain another.
It is revealed during Desert Treasure II that the relic is one of the three artefacts Azzanadra had been searching for in order to contact Zaros, alongside the relic and the Frostenhorn.
Trivia
- The icon slightly resembles the stereotypical version of a Hotei statuette, as it resembles a short, obese man sitting cross-legged while hands clasped together in a praying stance.
