The fairy mafia is a criminal group of fairies who have taken over Zanaris, led by the Fairy Godfather. Other members include Slim Louie, Fat Rocco, and many orks, who patrol the area when the Godfather is in power. After taking over the city, they violently attacked many of the fairies, including ripping off some of their wings.

In the Fairytale quest series, the player discovers that the original queen of Zanaris was set up to take heavy damage from a monster known as the Tanglefoot, which she and the Godfather were supposed to fight together. After letting the Tanglefoot incapacitate her, the Godfather temporarily became the leader of Zanaris, where he remains to this day; however, at the end of the second quest, the player is able to successfully revive the queen, who starts to prepare for war against the mafia.

Trivia

• The fairies in the mafia are based on depictions of the Italian-American Mafia in popular entertainment, including more direct references to films such as The Godfather.
• In RuneScape, a sequel to the quest called A Fairy Tale III - Battle at Ork's Rift sees the player help the queen return to the throne.
