The broken coffin can be obtained from Dampe in the town of Mort'ton. The coffin can be upgraded by giving locks found from the Shades of Mort'ton minigame to Dampe. Any lock can be added to the coffin at any point; lower tier locks do not need to be added to the coffin in order to upgrade it to the next level. You can only receive one coffin from Dampe. If you already have one, either a broken one or one with a lock attached, he will refuse to give you another one.
