The rotten apple is a quest item used only in Biohazard and Mourning's End Part I. Although they have an "Eat" option, rotten apples may not be eaten, attempting to do so will result in the message Nothing interesting happens..

In Biohazard, it is used to poison the stew to enter the Mourner's Headquarters while wearing a medical gown.

They are aesthetically similar to rotten tomatoes, although appear duller in colour and differ slightly in shape. Duplicates can be obtained after completing the quests, but it serves no further use.

They are found on the ground in north-eastern corner of West Ardougne, in a barrel near the port and several other places on Lunar Isle. There is also an orchard containing rotten apples directly east of the gated entranced to the Tree Gnome Stronghold. Players cannot pick rotten apples from the trees, though they may be found on the ground nearby in a Rotten Apple Pile.
