The strangled are humans infected with the Strangler plague and are exclusively found in The Stranglewood. They are encountered during the Desert Treasure II - The Fallen Empire quest.

They attack with typeless ranged that deals between 1 and 3 damage per hit. The attacks also have a chance to bind the player in place for a few seconds. Attacks will progress the player towards 100% infection; when this occurs, the player blacks out and returns to a previous checkpoint. During the quest, the player produces and consumes an advanced serum which renders them immune to infection and binding; however, damage will still occur from the attacks.

Strangled have a unique aggression mechanic in that if one strangled spots a player near them, nearby strangled will be alerted to their presence. This is indicated with the spotting strangled moaning Nngh..., and shambling towards the player; others alerted by that strangled will yell ! instead and dash towards the intruder. This additional aggression applies when other Strangled are within visual range of the initial aggressor. If the player steps or runs over a vine, this will alert more Strangled at a far greater range, even if they are not in visual range of the player. Strangled alerted by a strangler vine will always run at the player, and at high speed.

They also have a very high aggression range, with some of them being able to chase players into the Stranglewood mine. The range at which the strangled will notice the player can be reduced by walking rather than running while in their vicinity.
