Phosani's Nightmare is a more difficult version of The Nightmare, encountered by selecting Drink-from on the Pool of Nightmares in the Sisterhood Sanctuary.
Phosani's Nightmare can only be fought alone, and the game encourages the player to have at least one normal Nightmare kill before attempting Phosani's Nightmare; trying to attempt the boss without any Nightmare kill count will result in an initial warning advising the player to do so, though they may ignore it and proceed to the boss fight.
Compared to fighting the Nightmare solo, Phosani's Nightmare is significantly faster, with little damage being taken if players master the fight's mechanics, though far more punishing when mistakes are made. Although the boss itself has a lower drop rate for uniques compared to the original, it makes up for it with its faster kill times and increased quantities of normal drops.[1] It also possesses two unique drops the original lacks; the Slepey tablet, which allows players to access the Nightmare even quicker, and the parasitic egg, an item that unlocks a cosmetic skin for the little nightmare.
Difference in mechanics
There are notable differences in mechanics compared to the regular Nightmare encounter:
- Standard attacks hit significantly higher, but successfully praying against them will fully block the attack.
- Special attacks and minions are stronger and some are used in combination with one another: for example, spores are always summoned before six consecutive claw attacks.
- Phosani's Nightmare will use multiple consecutive claw attacks in conjunction with Flower Power or Spores.
- The spores slow the player's attack speed by 2 ticks instead of 1, and they also drain 6 prayer points every time the player yawns.
- Crush weapons will always deal their maximum hit against husks and parasites.
- There are five phases instead of three; 4 phases of 400 shield durability and 150 actual health, along with 4 sets of totems (16 in total) with 200 charge. In total, the player has to deal 1,750 physical damage and 1,600 charge damage (using magic) for a total of 3,350 damage in Phosani's encounter, compared to 6,600 (3 phases of 2,000 shield durability and 600 charge damage (using magic) damage in the original; making kills ~55% faster on average.
- After the end of each phase, the number of sleepwalkers spawned increases by one, capping at 4 at the end of the fourth phase, compared to a single sleepwalker at the end of all phases.
- Phases 1-4 use a set rotation of specials taken from the original encounter.
- In phase 4, Phosani's Nightmare will deactivate the player's protection prayer if they had already had the prayer active before the attack. She also has an increased chance of attacking with melee.
- In phase 5, the "desperation phase", she indefinitely performs Grasping Claws and summons sleepwalkers until killed. One sleepwalker is summoned at a time; should they reach her, she will unleash a blast causing 15 damage.

With each kill, Phosani's Nightmare has a chance to roll two unique drop tables, one containing the armour, mace, and staff, and one containing the orbs; both table rolls can be successful with each kill. The armour, mace, and staff drop table has a ~1/143 chance to roll, and the orb drop table has a ~1/533 chance to roll (with an effective chance of ~1/113 to receive any item).

Historical
Phosani's Nightmare was initially released as a competition lasting one week from 5 March 2020, allowing players to win free membership and signed artwork. Like the currently existing Phosani's Nightmare, it had more punishing mechanics and could only be fought solo, although it did not have reduced health, reduced damage when fought correctly, or phase randomisation. Its loot table was unchanged from the regular encounter.
- Its standard magic and ranged attacks could hit up to 90, while its slashing attack could hit up to 115.
- The Grasping Claws and Surge attack would instantly kill the player if hit.
- The spores would slow attack speed by 2 ticks, and last longer.
- Two sleepwalkers spawned instead of one.
- The husks were much more accurate and dealt more damage.
- Parasites could heal Phosani's Nightmare over 100 health, even when weakened. In addition, the damage taken when it burst out was 10 instead of 5, even when weakened.
- The Curse attack shuffled the protection prayers by one to the left rather than the right: clicking on Protect from Magic activated Protect from Melee, Protect from Missiles activated Protect from Magic and Protect from Melee activated Protect from Missiles.
