The Blood Altar (or true Blood Altar) is a runic altar used to craft blood runes from pure essence. Crafting blood runes at this altar provides 10.5 Runecraft experience per essence imbued. Level 77 Runecraft and completion of the quest Sins of the Father is required to craft blood runes.

Players may create a blood tiara here by using either a blood talisman or a tiara on the altar whilst having both in their inventory, granting 52.5 Runecraft experience. A catalytic tiara can also be created here by using a catalytic talisman and gold tiara, granting 42.5 Runecraft experience.

A second Blood Altar exists in Great Kourend. It was created by the power from the Dark Altar located near it, which draws energy from the plane where the true Blood Altar is located. Kourend's Blood Altar does not require a blood talisman to access.

Location

Routes to the Blood Altar via the Myreque Hideout.

The Blood Altar can be accessed from the trapdoor behind the Hair of the Dog tavern in Canifis or via fairy ring code DLS, requiring In Search of the Myreque, to be completed for quicker access to the hideout. Enter the cavern containing the tomb of Ivandis and take the other exit to arrive at the end of the Meiyerditch Laboratories. Players with level 74 Agility can access a shortcut at the end of the first path that takes them to the altar's entrance.

Players with level 93 Agility and 78 Mining, both of which can be boosted, can access a superior shortcut directly from the southern end of the Myreque Hideout from the tavern that takes them to west of the chamber containing the altar's entrance. However, players must mine their way through the strange wall in the laboratory tunnels to permanently unlock access to it.

To enter the mysterious ruins, players either have to use a blood talisman on the ruins, or click "Enter" on the ruins while wearing a blood tiara. Alternatively, a catalytic talisman or tiara can be used.

The Blood Altar can also be reached via the Abyss; the blood rift within the abyss is directed to both the true Blood Altar in Morytania and the false altar in Kourend.

Strategy

For high-level true Blood Altar runs while not using the Abyss, the following is recommended: a fairy ring teleport (such as the quest point cape), an equipable bank teleport (such as the Crafting cape), the colossal pouch, and completion of Lunar Diplomacy for the NPC Contact spell to periodically repair the pouch. Players should also have a steady supply of stamina potions to restore their run energy; a ring of endurance extends time between potion usages significantly. Those with a Runecrafting cape may wish to wear an Enchanted Lyre(i) or other non-cape teleport in order to prevent essence pouches from degrading.

The optimal setup is as follows:

- Construction Cape: fairy ring teleport in one's POH

- Crafting cape or other equipable bank teleport

- Colossal Pouch

- Access to the NPC Contact spell

- Full Raiments of the Eye outfit

- Blood essence

The ruins leading to the true Blood Altar were added to the game, but the altar itself remained inaccessible.

Trivia

The giant blood crystal from which the Dark Altar draws the energy to create Kourend's Blood Altar. Jovan's corpse can be seen next to it.
  • At the Blood Altar, a giant crystal can be seen discharging energy upwards. This is where the Dark Altar draws the energy used to create Kourend's Blood Altar.
  • Examining the skeleton in front of the aforementioned crystal results in the text "A message carved into the ground next to it reads: 'Minas, I'm sorry.'"
    • This skeleton belongs to a miner named Jovan, who went mad after coming into contact with a blood talisman during excavations in the Meiyerditch Laboratories, and subsequently stabbed Minas, who wrote the tatty note that can be found by searching his skeleton.
  • On the western and eastern side of the Blood Altar, players can use the oculus orb to view inaccessible templates of the Drill Demon random event area and Clan Wars, respectively. This is because the Blood Altar, like all areas not on the overworld map, is actually located on a section of the world map above the Wilderness which houses all of the underground and other non-surface areas of the game. Drill Demon and Clan Wars instances are generated from the templates seen via the Blood Altar.
  • In RuneScape, the Blood Altar was released on 22 April 2008 alongside Legacy of Seergaze. Although the mysterious ruins leading to the true Blood Altar were added to Old School on 4 June 2020 when Sins of the Father was released, it was not made accessible at the time due to the false Blood Altar already existing in Kourend. It was made accessible on 23 March 2022 alongside the addition of the blood talisman as a reward from the Guardians of the Rift reward shop.
  • Upon entering the true Blood Altar, the music track Bloodbath plays. This was the same music track used for RuneScape's Blood Altar in 2008; in Old School it was also used for the Slepe area.
  • Inside the altar, the shape of the main island is a blood drop, which bears the same symbol as the one on the blood rune.
