The blood talisman is a talisman that allows players to enter the Blood Altar where blood runes are crafted. The Blood Altar is located in the Meiyerditch Laboratories, which is discovered during Sins of the Father. Making blood runes requires level 77 Runecraft. The talisman is not required to access Kourend's Blood Altar, as it is not the true Blood Altar.

Blood talismans can be obtained from the Temple Supplies shop at the Guardians of the Rift minigame, granted the player has completed Sins of the Father. It can be combined with a tiara to make a blood tiara for 52.5 Runecraft experience at the Blood Altar, by using the tiara on the altar. This makes the talisman disappear, but the blood tiara can be used in just the same way and can be worn on the head, saving an inventory space when crafting blood runes.

Though it has a locate option, using it will give the message,The talisman doesn't seem to work. Maybe the ruins are deep underground. This message will persist even if the player is in the Meiyerditch Laboratories.

Trivia

  • The blood talisman is referenced by the dwarf Thorgel in the Mourning's End Part II quest. Players take a list of items to him in exchange for a death talisman, but if you ask why why he needs one of the three keys he asks you for, he will reply "Pretty please with a Blood Talisman on top?!", then you would say "A Blood Talisman?!", and then Thorgel would reply "Oops! I meant 'Death Talisman'!" with a shifty look on his face.
  • The blood talisman is an extremely rare example of an item that was present in the launch version of Old School RuneScape (in the game cache as an unobtainable item), removed from the game, and later re-added in a different update.
    • It was initially removed due to the false Blood Altar (which does not require a blood talisman) being added alongside the release of Great Kourend, and it became accessible in-game for the first time with the release of Guardians of the Rift on 23 March 2022.
